THE SCHOLARSHIP APPLICATION:
The Urbana High School PTSA scholarship is one that has been presented each year to an exceptional UHS graduate. The scholarship is available to all UHS graduates who will be pursuing post-secondary education. This scholarship is awarded to a student who values and demonstrates excellence in academics, a commitment to community service and has represented their community in a responsible, compassionate and ethical manner.
Criteria and General Information
- All graduating students are eligible to apply with the exception of children of the UHS PTSA Executive Committee and the UHS PTSA Scholarship Committee
- Only completed applications with all required attachments will be considered
- All information provided to the UHS PTSA Scholarship Committee will be held in strict confidence and will be shared only with the UHS PTSA Scholarship Committee and the UHS PTSA President
Timeline
- Applications will be available on the UHS website or in the UHS Guidance Office
- Completed applications with all attachments are due to Mrs. Kidd, in the Guidance Office, no later than May 18, 2009
- Award will be presented at the annual UHS Salute to Seniors ceremony
How to Apply
- Complete application.
- Include the following:
- Your mid-year transcript
- A listing of extracurricular activities/service activities/employment
- A listing of any awards/commendations, etc.(include academic, athletic, community service, extracurricular)
- One letter of recommendation
Submit ALL documents to Mrs. Kidd, in the Guidance Office,
by May 18, 2009
